The giveaway is the shape of the damage. Carpet beetle punches small, clean, sharply defined holes; carpet moth grazes an area into a thinning patch. If you are finding discrete holes in wool carpet or clothing, plus bristly banded larvae or the pale skins they shed, this is the right page.

And the reason it comes back after the carpet has been treated is almost always that the source was never on the carpet.
